FBFSEU members or other (in event of death of member)
Eligibility Criteria
1. Must be a member of the FBSEU.
2. Death of either parent or Legal Guardian of the member.
3. Death of member
4. Death of Spouse
5. Death of member’s children
Note: 1. Two applications only for parents’ death – Father and Mother whether guardian or biological.
Pre-Qualification Requirement
1. Written advice by member
2. Written advice from Union rep.
3. HR or Union rep confirmation that member is single (if applicable)
4. Death Benefit Application Approval Form
5. Death Certificate for deceased. Notification of death followed by the Death Certification.
6. Legal Guardian – Statutory Declaration signed by a lawyer confirming legal guardian.
7. Birth certificate in case of children, Marriage certificate in case of spouse or Member (if applicable)
8. ID of person collecting benefit. Confirmation by Union Rep who the benefit will be paid to and method of payment and Bank Account details required. Authority by member if collection by third party.
Eligibility Amount (based on continuous years of union membership)
Member(s) Death – $1000 plus $50 for every year of membership
Spouse and Children – $1000
Single Member (Death of a parent) – $1000
Married members Parents/Legal Guardian – $500
Exclusions
1. Parents (in-laws) of members are not eligible.
2. Any violation of the FBSEU policies and constitution.
3. Members that are in 4 weeks in arears of subscription shall fully clear his/her arears before claiming member benefits.
4. Members that have more than 4 weeks in areas are not eligible for member benefits.
5. New Members joining the FBFSEU and wanting to claim the benefit on the basis of the 1-year claim but were not an FBFSEU member at the time of the event.

Eligibility Criteria
1. Must be a member of the FBFSEU.
2. Have reached retirement or early retirement or redundancy or their position become de-established by their employer.
3. Age between 45-55 years old.
4. For those that are 55 years and over become eligible at the end of their employment.
5. With exception to members retiring on medical ground before reaching the age cap – $500 irrespective of membership term.
Note: Members who have left the FBFSEU and wanting to rejoin again, their membership will start again from being a new member and their previous membership years will not accumulate from when they left.
Pre-Qualification Requirement
1. Retirement or early retirement or redundancy letter from organization.
2. Confirmation by Union Rep who the benefit will be paid to and method of payment and Bank Account details required. Authority by member if collection by third party.
Eligibility Amount (based on continuous years of union membership)
5 – 10 years – $500
11 – 15 years – $750
16 – 20 years – $1000
21 years plus – $2000 + a token
Exclusions
1. One-off retirement benefit payment and no further retirement benefit.
2. Any violation of the FBSEU policies and constitution.
3. Members that are in 4 weeks in arears of subscription shall fully clear his/her arears before claiming member benefits.
4. Members that have more than 4 weeks in arears are not eligible for member benefits.
5. New Members joining the FBFSEU and wanting to claim the benefit on the basis of the 1-year benefit claim but were not an FBFSEU member at the time of the event.

Eligibility Criteria
1.Must be a member of the FBFSEU.
2.Disaster of any form. Natural (geophysical, hydrological, climatological or meteorological disasters) or man-made (fires)
Pre-Qualification Requirement
1. Advice by member or Union rep.
2. Extent of damage and, or evidence of disaster (e.g. Police/NFA report – Fire) and as requested under cyclone damage.
3. Confirmation by Union Rep – who the benefit will be paid to and method of payment and Bank Account details required. Authority by member if collection by third party.
4. Disaster Assessment Form (Assessment officer and member to sign off assessment report)
Eligibility Amount
On merit – maximum of $1000 per annum per applicant
Cyclone, Earthquake and Tsunami – $250 Secretariat limit.
Floods – $250 Secretariat limit.
Fire – $250 Secretariat limit.
Exclusions
1. Any violation of the FBSEU policies and constitution.
2. Members that are in 4 weeks in arears of subscription shall fully clear his/her arears before claiming member benefits.
3. Members that have more than 4 weeks in arears are not eligible for member benefits.

1. Evidence of participation and endorsement from National Sports Federation endorsement
2. Confirmation by Union Rep or member on method of payment and Bank account details required. Authority by member if collection by third party.
Eligibility Amount
Max $500 per annum per member
Exclusions
1. Any violation of the FBSEU policies.
2. Members that are in 4 weeks in arrears of subscription shall fully clear his/her arrears before claiming member benefits.
3. Members that have more than 4 weeks in arrears are not eligible for member benefits.
4. New Members joining the FBFSEU and wanting to claim the benefit on the basis of the 1-year benefit claim but were not FBFSEU member at the time of the sporting event.
